Ottawa & Delaware Counties Community Health needs assessment survey with INTEGRIS HEALTH and NORTHEASTERN TRIBAL HEALTH SYSTEM
Integris Health has been charged with completing community needs assessments in each of the areas they have a presence. The Community Wellness Committee of Northeastern Tribal Health System was asked to assist in the distribution of the surveys. The data collected from the surveys will be analyzed to determine the 5 most significant issues impacting the health of each county.
Below, you will find links for Ottawa & Delaware County adult (18 & over) residents. It is important that we work together to distribute this survey to as many applicable people as possible-think friends, relatives, neighbors, co-workers, church members, club/organization members......The more individuals that we are able to get to complete the survey, the more confident we can be that the results are TRULY representative of our population, and that the health improvement plan that is developed will benefit the greatest number of people within our communities residing in Ottawa & Delaware Counties.
